Information for applicants

Reports on the results of an autopsy and the cause of death cannot be made over the phone. You can only tell us by phone whether the deceased was autopsied at the ÚSL, and the name and contact information of the autopsy doctor.

Performing an autopsy

The decision to perform a medical autopsy is made by the examining physician by making a note in the deceased's documentation. A forensic autopsy is ordered by the criminal prosecution authority in accordance with applicable legal regulations. If there is no indication for a medical autopsy from a medical point of view, the autopsy does not have to be performed. The decision on this action is made by the chief physician of the ÚSL.

News for Survivors (Close)

According to the law, persons close to the deceased have the right to inspect, extract or copy medical records. According to Section 116 of the Civil Code, a close person means direct relatives, siblings, a spouse and a registered partner of a specific individual. Other persons in a family or similar relationship (grandparents, uncle, aunt, brother-in-law, cousin, cousin, kind, companion) are considered close to each other if the other would reasonably consider the damage suffered by the other to be his own. The applicant must prove that he / she is close (identity card, affidavit stating his / her contact details and identity card number, copy of marriage certificate, copy of birth certificate)

Responses to written requests for information addressed directly to the ÚSL with credible evidence that the applicant is a close person are sent directly to the applicant. If there are doubts about the credibility of the evidence that the applicant is a close person, the request is forwarded to the Legislative and Legal Department of the General Hospital. Responses to written requests for information addressed through the Legislative and Legal Department are sent back to the applicant through the Legislative and Legal Department. Information about the results of the autopsy is not provided to close persons if a judicial autopsy was performed pursuant to Section 115 of the Criminal Code. If the case is not finally closed, the conclusions are not communicated.

Written reports

The applicant must submit written request. The completed application is must be attached:

• proof that the applicant is a close person or legal representative of the deceased
• copies of two identity documents of the applicant (identity card, birth certificate, passport, etc.)

The request for a copy of the medical documentation, including proof of the relationship with the deceased, is based on the autopsy report.

The end of the autopsy stating the cause of death, the underlying disease, complications and the secondary finding, the applicant will receive a written form to the address specified in the application. The service is paid according to VFN price list on unpaid health services.

Payments

• Payment only on delivery upon receipt of the shipment. We only ship to addresses in the Czech Republic.

Report to the attending physician

• If the attending physician is interested in the autopsy result, it is sent to a written request.

Death certificate, death certificate

• These documents are not issued at the ÚSL.
• The death certificate is exposed to the catchment matrix and passed to the survivors approximately 2-3 weeks after the death.

Valuables, clothing - deceased persons outside the VFN

Valuables, including dentures made of precious metals, found on the deceased are recorded by the examining physician in the accompanying documentation of the deceased. When the body is received at the ÚSL, they are recorded in the deceased's Clothing List and subsequently placed in a lockable box. When removed, they are handed over to the funeral service, which arranges the funeral.

The clothes for the deceased in the coffin can be handed over to the funeral service, which arranges the funeral. The garment in which the deceased was brought for autopsy is handed over to the funeral service together with the body. In case of biological contamination of the garment, the autopsy will decide on its disposal.

Exposure of the body

It is not possible to display the body on the premises of the ÚSL. Unless the autopsy doctor determines otherwise, the display of the body after the autopsy is within the capabilities and competence of the funeral service with which the funeral was arranged.

Ordering a funeral

The choice of funeral service is entirely up to the bereaved. ÚSL employees are not authorized to interfere in the choice. To order a funeral, it is necessary to present the deceased's ID card (birth certificate) and the ID card of the person arranging the funeral to the funeral home. Laboratory examination

At the Institute of Forensic Medicine and Toxicology, blood alcohol levels are determined for living persons for the needs of the Czech Police and other bodies.

Price list

• Prices are charged according to the Price List of Services not covered by health insurance in accordance with VFN legislation.
• Blood / urine tests for alcohol: according to VFN price list.

Payments

• By postal order, in cash at the VFN box office.
• The result will only be issued to the applicant after proof of payment has been provided.
