Transition group
The Transition Group is an outpatient program that forms an integral part of the basic treatment model at the Addiction Clinic. It has two parts, residential (inpatient) and outpatient (attendance). The Transition Group helps patients safely manage the adaptation from the protected environment of residential treatment to everyday life and maintain the changes they have achieved during this first phase of treatment.
PROGRAM OBJECTIVE
The goal is to support stability during the period when relapse is most likely. The program combines professional guidance with practical training in everyday life skills:

maintaining abstinence and strengthening resistance to relapse

managing cravings and risky situations
-
development of the ability to manage one's own life (self-management)
-
connection to other forms of care according to individual needs
HOW THE GROUP WORKS
The program takes place in a semi-closed group, which allows for safe sharing of experiences and targeted therapeutic work.

patients after completion of inpatient treatment (4–8 weeks)

mandatory for clients from Prague and the Central Bohemian Region

In the event of relapse, returning to another form of care is recommended.
WHY JOIN THE GROUP
Returning to normal life after treatment brings new challenges. A transition group offers a safe space for sharing, finding solutions, and building confidence. Regular support significantly reduces the risk of relapse and helps maintain long-term change.
