The center of heart failure is ambulance II. internal clinics specializing in the treatment of patients with heart failure. In addition to outpatient examination by a cardiologist dealing with this issue, we offer the possibility of echocardiographic examination on a device located in the center, a day hospital for patients with the possibility of outpatient administration of some drugs intravenously or some treatment for outpatients (eg chest puncture, long-term intravenous catheter) in cooperation with the VFN Pain Center) without the need for hospitalization of these patients in a hospital bed. Of course, there is a link to the complement of the clinic with the availability of a laboratory and X-ray examination in the building.
Outside the office hours of the outpatient clinic, it is possible to consult patients with the doctors of Coronary Unit II. internal clinics of the General Hospital.
The department also participates in research projects both within the framework of clinical grants and commercial projects, especially within the research of new drugs or treatment procedures.

Spectrum and service offering
The Center for Pulmonary Hypertension is the only one in the Czech Republic that deals with the comprehensive diagnosis and treatment of all types of chronic pulmonary hypertension, in particular:
- Pulmonary arterial hypertension including specific treatment,
- chronic thromboembolic pulmonary hypertension including surgical treatment (pulmonary endarterectomy) and catheterization (balloon pulmonary angioplasty) for the entire Czech Republic and Slovakia.
Eligible patients
- Patients with a moderate or high probability of pulmonary hypertension on echocardiography that is not explained by significant cardiac or pulmonary disease
- Patients should ideally be referred to the center with the results of an echocardiographic examination, pulmonary function tests, and ventilation-perfusion scintigraphy.
About the workplace
Center for Pulmonary Hypertension at the General University Hospital in Prague is the largest expert center for pulmonary hypertension in the Czech Republic. It operates within the Cardiocenter of the General University Hospital. It is dedicated to all types of chronic pulmonary hypertension. It is the largest workplace for the diagnosis and treatment of pulmonary arterial hypertension in the Czech Republic. It is the only one that deals with the comprehensive treatment of chronic thromboembolic pulmonary hypertension (CTEPH). Since 2004, more than 400 pulmonary endarterectomies have been performed at the Cardiocenter of the General University Hospital with results comparable to top foreign workplaces. The workplace also provides this treatment to patients referred from abroad (Slovakia, Romania). In addition to surgical and pharmacological treatment, the center has been intensively developing percutaneous treatment of CTEPH using balloon angioplasty in recent years. By the end of 2021, 330 of these procedures had been performed here.
Since 2017, the Center for Pulmonary Hypertension at the General University Hospital in Prague has been among the 17 European expert centers that are part of the European Reference Network (European Reference Network) focused on the issue of pulmonary hypertension. By decision of the Ministry of Health of the Czech Republic, the center has had the status of a Center for Highly Specialized Care for Rare Respiratory Diseases since 2022.

The clinic has a long tradition in the diagnosis and treatment of cardiac arrhythmias and is one of the established workplaces in this field. Outpatient examinations, consultation activities or dispensary visits take place both in the premises of the cardiostimulation clinic on the premises of the clinic and in the arrhythmia clinic at the Faculty Polyclinic on Charles Square.
We are currently able to provide comprehensive care for patients with all types of arrhythmias and tachyarrhythmias.
Most patients with arrhythmia problems, especially those after more demanding catheterization procedures, are hospitalized in the clinic's Antiarrhythmic Unit. The department has nine monitored beds.

Pacing, ICD implantation and cardiac resynchronization therapy
Pacing is now the dominant method of treating patients with bradyarrhythmias. It has been performed at the clinic since 1974. Implantable cardioverter-defibrillators have been introduced at the clinic since 1994, and we have been performing resynchronization therapy for heart failure since 2000. Patients who have had pacemaker implants continue to be monitored for pacing issues at regular intervals in the routine pacing clinic.

Catheter ablation of cardiac arrhythmias
The clinic performs catheter ablation for the entire spectrum of heart rhythm disorders: procedures for common supraventricular tachycardia, atrial fibrillation and associated atrial tachycardia (including type I atrial flutter), ventricular tachycardia, both idiopathic and associated with structural heart disease. The Fabry Disease Center is dedicated to the diagnosis and treatment of patients with this rare metabolic disease. It closely cooperates with other specialized departments of the General University Hospital and the First Faculty of Medicine of Charles University, for example, with the Institute of Inherited Metabolic Disorders, the Department of Pediatric and Adolescent Medicine, and the Center for Cardiomyopathies. The Fabry Disease Center also concentrates nationwide care for patients with this hereditary multi-organ disease.
All patients suffering from Fabry disease are comprehensively examined on an outpatient or hospitalized basis and continue to be monitored regularly in our specialized outpatient clinic.
The center also indicates the initiation of specific therapy. The use of this therapy, if indicated in time before the development of advanced renal insufficiency, leads to a reduction in pain, an improvement in quality of life, a slowdown in the progression of left ventricular hypertrophy, and above all, the stabilization of renal function in our patients. One of the options is enzyme replacement therapy (ERT), administered in the form of regular infusions; some patients receive treatment directly in our inpatient unit, while patients from outside Prague receive treatment at their place of residence. For suitable patients, we provide oral treatment. Conventional treatment of the disease also includes pain relief with analgesics, nephroprotection (angiotensin-converting enzyme inhibitors and angiotensin receptor blockers), the administration of antiarrhythmic drugs or pacemaker implantation, dialysis and kidney transplantation.
We currently care for more than 180 patients, making us a world leader in this field.

The Working Group of Myocardial and Pericardial Diseases performs comprehensive diagnostics and treatment of all types of myocardial and pericardial diseases, including rare metabolic and neuromuscular diseases. In this area, it cooperates with other specialized departments of the General University Hospital and the 1st Faculty of Medicine of Charles University: Radiodiagnostic Clinic, Institute of Nuclear Medicine, Institute of Inherited Metabolic Disorders, Clinic of Pediatric and Adolescent Medicine, Clinic of Neurology, Institute of Biology and Medical Genetics, and Laboratory of Molecular Cardiology.
The unique programs of our center include diagnostics and specific treatment of inflammatory cardiomyopathy (including biopsy diagnostics), cardiac amyloidosis, sarcomeric forms of hypertrophic cardiomyopathy and their phenocopies (in symptomatic obstructive forms, alcohol septal ablation or surgical myectomy is available for patients), and diagnostics and treatment of constrictive pericarditis.
As one of the few in the Czech Republic, our department also provides a program of routine genetic diagnostics of cardiomyopathies.
Most patients suffering from myocardial and pericardial diseases who were examined at the clinic as an outpatient or during hospitalization continue to be regularly monitored in a specialized outpatient clinic, in permanent cooperation with primary treating outpatient cardiologists.
An integral part of the activities of the working group on myocardial and pericardial diseases is the presentation of its own scientific observations at domestic and foreign cardiology congresses and in Czech and foreign professional journals.

Warfarin should be discontinued 3 days before admission, new anticoagulants such as Pradaxa, Eliquis, Xarelto should be discontinued the day before admission. Do not discontinue acetylsalicylic acid, clopidogrel, prasugrel, or ticagrelor.

Intensive cardiac care at the 2nd Internal Medicine Clinic is based on a rich tradition. With its scope, which includes a coronary unit with a total of 8+1 beds, an intermediate/antiarrhythmic unit with 9+1 beds and an intensive angiological care unit with 10 beds, it provides a sufficient bed base for the care of critically ill patients with cardiological and angiological problems in all phases of acute illness.
All beds are equipped with a modern monitoring system with the possibility of continuous 12-lead ECG recording, Holter monitoring with analysis of heart rhythm disorders and complete invasive hemodynamic monitoring. The coronary unit also has three lung ventilators, a device for mechanical support of blood circulation by intra-aortic balloon counterpulsation, one device for performing extracorporeal purification methods (CVVH, CVVHD, CVVHDF). There is also a sufficient number of modern external pacemakers available, including one dual-chamber for the possibility of temporary AV sequential cardiac stimulation for patients with rhythm disorders.
The coronary unit, as one of the key departments of the Comprehensive Cardiology Center of the VFN, also provides care for patients requiring mechanical circulatory support, primarily using the ECMO method (extracorporeal membrane oxygenation) and percutaneous left-sided Impella support.
Mechanical cardiac support is used primarily in patients with acute myocardial infarction with cardiogenic shock and in patients with refractory circulatory arrest. The ECMO program for refractory circulatory arrest, both in-hospital and out-of-hospital, is one of the dominant and unique programs of the entire cardiocenter.
With the above technical background, the coronary unit team mainly cares for patients with acute coronary syndromes, complicated patients in cardiogenic shock, with lung and kidney failure, patients after cardiopulmonary cerebral resuscitation, patients with malignant arrhythmias and, last but not least, patients after complex catheterization procedures. Within the Cardiocenter of the VFN, we of course cooperate closely with the Department of Cardiovascular Surgery and the Department of Anesthesia, Resuscitation and Intensive Care Medicine, which form the Comprehensive Cardiocenter of the VFN.
A number of research projects are underway within intensive cardiology care, especially in patients with myocardial infarction, circulatory arrest, and the need for mechanical circulatory support.

The vascular angioline peripheral angiography offers a complete spectrum of diagnostic and interventional procedures in angiology.
Approximately 1,300 diagnostic catheterizations and nearly 850 interventional procedures on the arteries and venous system are performed each year. A significant proportion of patients are examined and treated for venous problems, including acute conditions, which are the same as arterial procedures are performed in continuous operation. As in the coronary artery, interventional arteries and veins are treated with percutaneous transluminal angioplasty or stent implantation. Various types of catheterized thrombectomies are performed and cavalry filters are implanted there. In the area of local thrombolytic treatment of arterial and venous occlusions, the workplace has priority not only from the national point of view.
In cooperation with the Gynecology and Obstetrics Clinic, we provide therapy for patients with pelvic congestion syndrome, and we also cooperate with the Urology Clinic and perform interventional treatment of varicocele in men with fertility disorders.

The Department of Non-Invasive Angiology of the 2nd Internal Medicine Clinic consists of a vascular outpatient clinic and a vascular ultrasound laboratory in the main building of the clinic, with another workplace at the Faculty Polyclinic.
The main task of the ambulance and sonography is diagnostics of vascular diseases and proposal of further examination and treatment proceduresMany of those examined are referred for interventional or vascular surgery. We continue to monitor complicated patients on an outpatient basis and manage their long-term treatment.
We specialize in a wide range of vascular diseases. We use a variety of methods to make a diagnosis - from treadmill stress tests and simple Doppler examinations to modern imaging methods such as duplex sonography, computed tomography and nuclear magnetic resonance (in cooperation with the Radiodiagnostic Clinic).
Peripheral arterial disease is a widespread problem, but diseases due to atherosclerosis clearly predominate. We also treat associated diseases that lead to atherosclerosis (e.g. hypertension, diabetes, high fat levels, etc.). We also diagnose and treat rarer, non-atherosclerotic arterial disorders.
Venous problems are mainly represented by deep vein thrombosis and its complications – pulmonary embolism and chronic venous insufficiency. We cooperate with the wound healing clinic and vascular surgeons performing radical treatment of varicose veins.
Vascular ultrasound is a modern examination method that allows for high-quality imaging of the vascular system of virtually the entire body. Most often, these are the arteries supplying the brain, the limb arteries, and the abdominal aorta; the venous system is examined mainly in the limbs.
The vascular clinic is connected to the ultrasound department, which allows for comprehensive and immediate diagnostics. It also forms an important "service link" for the operation of the clinic - it provides information on the condition of the patient's vessels, e.g. before planned heart surgeries. Possible complications of invasive procedures (e.g. closing post-catheterization pseudobulges by administering thrombin) are also addressed here.
Doctors of the ultrasound department participate in the undergraduate and postgraduate education of doctors.

The vascular outpatient clinic and sonography department are located on the ground floor of the clinic (entrance A14B), in the premises reserved for specialized outpatient clinics. They are open daily (Monday–Friday) from 8:30 a.m. to 3:30 p.m.
For initial examinations (based on the recommendation of a general practitioner or other specialist), as well as for follow-up examinations and ultrasounds, it is necessary to make an appointment in advance (optimally in the afternoon):

Our other workplace, which has similar possibilities for diagnosing and treating vascular diseases, is at the Faculty Polyclinic, Karlovo nám. 32, in building B, on the 4th floor.

The echocardiography laboratory provides comprehensive ultrasound diagnostic services within the Cardiology Center of the General University Hospital and for cooperating departments and physicians. Transthoracic (TTE) and transesophageal (TEE) examinations, including stress tests (physical and pharmacological stress), are routinely performed.
Echocardiographic examinations are performed daily, from 8:00 a.m. to 3:00 p.m. Approximately 8,000-9,000 echocardiographic examinations are performed in the laboratory annually.
The research efforts of the laboratory staff are focused primarily on strain rate imaging of the myocardium, 3D echocardiography, and monitoring of patients with unique diseases treated in specialized outpatient clinics of the clinic (see Center for Pulmonary Hypertension, Fabry Disease Center and Center for Myocardial and Pericardial Diseases). The results of the echocardiography laboratory's research work are presented annually at domestic and foreign professional events, and original works are published in prestigious medical journals (see clinic publications).
The echocardiography laboratory staff are the main organizers of the annual one-day symposium with international participation, PRAGUECHO, which is dedicated to current issues in echocardiology.

The II. Internal Clinic of Cardiology and Angiology offers comprehensive diagnostics and care for patients with valve defects. We then monitor patients with valve prostheses or after other valve procedures. We deal with the entire spectrum of both acquired and congenital heart defects and use the full spectrum of imaging methods. We have invasive and non-invasive examination methods available. Our main task is diagnostics and the design of further examination and treatment procedures. Many of those examined in this way are subsequently sent for surgery at cardiac surgery department our Cardiocenter or to interventional catheterization solution (TAVI, Mitraclip, balloon valvuloplasty). The entire process is in collaboration with a referral cardiologist and a general practitioner.
Patients with a detected valve defect, for whom surgical treatment is not yet necessary, are further monitored clinically and sonographically, depending on the severity of the defect and clinical condition, usually at intervals of 3 months to 1 year.
In the event that the patient is indicated for surgical or interventional treatment of the defect, a catheterization and preoperative examination are performed during hospitalization. For patients who are indicated for valve surgery, we perform: laboratory tests including blood type, coagulation parameters, biochemical parameters, urine examination. Furthermore, X-ray of the heart and lungs, examination of pulmonary functions and ultrasound examination of the carotids. To exclude an infectious focus, we supplement the ultrasound examination of the abdomen, X-ray examination of the paranasal sinuses with ENT examination, dental examination and, for women, gynecological examination. Most of these examinations can be supplemented on an outpatient basis after agreement with the patient.

Scope of possible cooperation, method of ordering, examination dates
Outpatient activities are focused on those who cannot be treated at another facility (pulmonary hypertension, Fabry disease, partially cardiomyopathy, etc.). Among general cardiology patients, we can significantly help patients with tachyarrhythmias, but it is also in the interest of these patients that they be booked into the specialized Arrhythmia Outpatient Clinic for a supercociliary examination and therefore recommended, preferably by an internist or directly by a cardiologist.
We are able to examine patients with general cardiological problems, but with the following:
- it is necessary to respect the appointment times, which are similar to those in other cardiology clinics in Prague,
- We must give priority to patients referred from other highly specialized VFN centers,
- We are only able to provide permanent dispensary care to a minority; rather, it is a one-time examination at a higher workplace with a recommendation for further action, including the optimal type of workplace where further care should be provided.
ECG curves are registered without an appointment upon recommendation (it is also appropriate to use the afternoon hours for registration, when the number of patients is lower and their waiting time is shorter), but we are limited in the possibilities of ECG registration and it is not within our capabilities to provide this service for doctors outside the VFN. For other auxiliary cardiological examination methods (as well as for outpatient clinics), an appointment must be made by calling 22496 6230 (or in person at the reception). The appointment dates vary depending on the individual methods and we try our best to keep them within a few weeks at most. We prefer to accommodate patients with recommendations from other VFN departments, but we do not object to one-off echocardiographic and other examinations with a clear request from extramural doctors.
Auxiliary cardiological methods available at the FP
Holter ECG examination
We perform 24-hour Holter ECG monitoring primarily for arrhythmological indications. The results of the examination appear in the hospital information system Medea within a week and are simultaneously sent in extenso to the address of the referring physician. The units are usually fitted to patients between 9 and 10 am and patients are asked to return them by 8 am.
Ambulatory blood pressure monitoring
24-hour blood pressure monitoring is provided by Spacelabs monitors at our workplace, which are considered to be of a high standard. The units are usually deployed between 10 and 11 a.m., and patients are usually asked to return around 8 a.m. the next day. The patient is given a record of the results for the doctor of the 2nd Internal Medicine Clinic (and the cooperating Clinic of Nephrology) immediately, other doctors receive a record with a verbal evaluation within 3-4 working days. If the referring doctor requests a printout without the need for a verbal description, we will of course comply. The Faculty Polyclinic also has an Apnea-link device for performing limited somnography.
Stress tests
Ergometry is usually performed on an Ergoline bicycle, but a treadmill can also be used to simulate the load. In both cases, the examination is evaluated by the Cardiosoft computer system. As part of the recent reconstruction of the department, a shower was built for the comfort of patients after the stress test. In the first half of 2011, the examination began spiroergometric.
Jogger
We routinely use the treadmill primarily to determine the claudication interval for patients with ischemic lower limb disease. It is indicated as such by intramural angiologists, but we do not oppose the requirements of cooperating angiologists. There is also a connection to a twelve-lead ECG with software for stress tests. The treadmill thus becomes an alternative to classic ergometry for patients with a special computer program that allows for stress testing of the heart on the treadmill. We use it for patients unable to exercise on a bicycle ergometer.

The operation of the 1-day inpatient unit (1DS) at the 2nd internal clinic was launched in November 2020. In accordance with modern trends in patient care, we are trying to streamline the time patients spend in the hospital and work very closely with outpatient doctors. The 1DS is available during working days, from Monday 7:00 a.m. to Saturday 7:00 a.m.
We have seven monitored beds where the patient is placed for 24 hours with a clear examination plan in advance. Patients come to us with appointments for the Cardiac Catheterization Room, the Vascular Catheterization Room, the Ablation and Stimulation Room, for electrical cardioversion, for effusion punctures or for a short examination stay.
Patients undergoing Cardiac catheterization room most often selective coronary angiography (SKG), which can be immediately followed by direct angioplasty with stent implantation (PCI). The radial approach is preferably used as the approach (via the radial artery on the thumb side of the wrist), which eliminates the need for several hours of immobilization of the patient in bed with groin compression. Approaches from the groin, the so-called femoral approach, via the femoral artery, are used if the patient has already had a coronary artery bypass graft or if this approach is necessary for other medical reasons. Patients come fasting, drinking clear liquids is not restricted before the examination. The patient is discharged to home care the next morning.
Patients coming for procedures at Vascular catheterization room They usually have to undergo a procedure in the groin, i.e. femoral, via the femoral artery, where after the procedure it is necessary to have several hours of compression of the punctured artery. Most often they undergo a simple angiographic examination of the arteries of the lower extremities (examination of the arteries of the lower extremities after filling with a contrast medium) or plasty of stenoses (narrowings), when an angioplasty procedure is performed at the same time along with stent implantation (PTA). After checking the groin, these patients go home the next day. Patients come fasting, drinking clear liquids is not limited before the examination. The patient is discharged to home care the next morning.
Patients coming for procedures at Ablation room they always have the procedure from the groin, from the femoral vein, not the artery. After the procedure, they have compression for several hours. The length of the procedure depends on the arrhythmia being intervened. Patients come fasting and do not drink fluids. The patient is discharged to home care the next morning.
Patients coming for implantation permanent pacemaker (TCS), its replacement, or implantation cardioverter (ICD) They must be fasting and not drink any fluids. Since the device is implanted in the subclavian area, they must rest for several hours after the procedure, especially the left upper limb. They are discharged the next morning.
Patients who come to electrical cardioversion they must take full anticoagulant therapy (Warfarin or Eliquis, Pradaxa or Xarelto), they take the medicine in the morning, then they remain fasting and do not drink fluids. The procedure is performed under general analgosedation of the patient. The patient is discharged home the following day after the procedure.
For other procedures, it depends on the type of procedure. The referring doctor will always carefully instruct the patient or you can call the phone number below. We send an informative SMS to all indicated patients on Friday of the previous week.
The staff caring for the patient at the 1DS also carefully educates the patient about the patient's diagnosis and other non-pharmacological options for improving the disease (they will recommend options for weight loss, smoking cessation, etc.). During hospitalization, it is also possible to use the services of psychological and addiction support.
The patient leaves the hospital with a clearly defined outpatient program or further treatment plan within the hospital.
